SSA STAR Care supports people with a wide range of mental health needs. These can include personality disorders, mood disorders, anxiety and obsessive-compulsive disorders, schizophrenia and dementia. Many who have chosen Achieve together to support them have endured a number of hospital admissions.
They may experience a range of additional life challenges including , poor physical health and relationship issues. Further life challenges are difficulties with addictions, sleep disorders, hoarding, self-neglect, self-harm and/or suicidal ideation.
We enable people with mental health needs to fulfil their potential on a long-term basis. Once Achieve together has been chosen to provide mental health support, individuals are involved in many aspects of planning their support which enables us to provide bespoke plans of support, and create person-centred risk, and crisis/relapse prevention plans. Within planning a lot of focus is on enhancing self-confidence, esteem and empowerment.
Our pro-active teams support initiatives to help people relearn the skills required for daily living to enhance independence and minimise support. We do so in collaboration with the person, and local statutory and community/voluntary services.
